A. Two visits to the home:
- First visit:
- 2 hours for assessment of dietary practice in the home and assessment of nutritional status for 5 residents in the home, watch residents eat.
- Second visit:
- 3-4 hour motivational presentation/workshop which includes weight management, portion sizes, frequency of meals and snacks, heart health-(high cholesterol and other lipids, high blood pressure), any other issue identified during the initial assessment.
- 4 week cycle and multicultural menu, balanced diet/meals with reference to Canada Food Guide
- Recipes and grocery list provided
- cooking demonstration with our chef, both staff and residents are welcome to join, how to use the menu, recipes and the grocery list.
- Certificate given to staff that attended and participated
- We provide written general nutrition recommendations for the home, and for the residents (clients) who need it.
- Resident's/individual's nutritional needs, dietary habits, medications etc taken into consideration
B. One 15-20 minutes telephone follow-up with nutrition staff six months later
C. Annual maintenance available
